2015-2016 Tryout Paperwork Due April 3

The MISD tryout meetings for next year have concluded. Residency verification (most completed this at the meetings) is due by February 20 to Dr. Sanders' office at The Center for Performing Arts. All other paperwork that was in the tryout packet is due April 3 to Mrs. Parlin at Legacy. Please make sure all forms are complete, and if required, notarized. A current physical must be included. The physical form is two pages, and is in the tryout packet. Please have the doctor complete both pages. You may emaill Mrs.Parlin or Mrs. Crook with any questions.
